Sizing forms/placing text??

Hi Jason,
Trying to build a small form for a home page (250px wide) and can't seem to:
1) get the size correct, the fields push over the right border.
2) want to get the question text above the field like this static layout, lower right of page. http://americanmadegoods.net/nelly/
that should make things fit?

Data Bridge uses Form Builder designs for creating the forms.

you can use form builder to create a preset that has the layout you are trying to achieve, in form builder, go to the design tab to create your design settings, when done you will be prompted to save the settings as a preset, you can then select the preset in Data Bridge.

on the design tab, select the Form Elements -> Labels section to change the label placement. you can select top / left to have the labels placed above the form elements.

in the form section, you can set the form width.
