I am having trouble inserting a record

I am having trouble inserting a record. I would also like to upload an image in one of those fields. I cant get it to upload anything.

Im sure you will have me sorted out quick fast and in a hurry. Thanks in advance.

For file upload to work, the forms encoding type must be set to multipart/form-data

in the tag selector, select the form tag, then in the property inspector, change the encoding type to multipart/form-data.

Thanks, however that didn't fix it

I cant find where to bind the values in the the insert dialog. The form doesn't come up to pick.
I changed what you said above but it still isnt working. Thanks Advance.

your form elements are not valid. for example:

<input name="order_num">


is missing the ID, type and value attributes:
<input name="order_num" id="order_num" type="text" value="">
