Have error messages from Momento as I open a page in Dreamweaver 5.5

These attachments are the errors I get and the order they come in. How can I fix these. I have just started using CS5.5 again because of all the problems I have with CS6. Now this.

reset the workspace layout:
go to Window -> Work Space layout -> Reset '<your layout name>'

and Delete the Dreamweaver Cache file from the users configuration folder:

Oops. looks like the dialogue box opens when I open a file now. Same 3 boxes as I sent before. this is after I followed your instruction.

CSS Form builder is not compatible with DW CS6, you will need to upgrade to Data Bridge.

Data Bridge contains a version of form builder that has been updated to be compatible with DW CS6. the stand alone form builder extension is not compatible, though.
