Security Assist can't see database table

I created register, login, logout, user update pages with Security Assist. I created a localhost database and a users table. I can add a user to the database users table through the register page, and I can log in with the login page, so the connection must be OK, although I had to type in the database name myself. I could not select or define it via Security Assist because, if I click on Select , it gives a dreamweaver message "An unidentified error has occurred". If I click test, the message is "Please specify a Database". And no table shows up under my Business Catalyst Databases window. I'm now on day 3 trying to solve this and get my site live, any help is appreciated!!!

the error message:
"An unidentified error has occurred".

means that there is a problem with the site definition in Dreamweaver for the testing Server connection

Double check the Site Testing server settings to make sure they are correct. see the following tutorial fore details on creating a Site in Dreamweaver:

Yup. I had the index page included in the URL. Deleted that and it works. Thank you. But... applying Security Assist Authenticate User has messed up the style of the login page - can't figure that one out yet...

Very quick responses! Thanks. Here's the remote site www.prosportscollectors.com. Also when I log in I get these errors (feel free to register & try it out):

I dont see a problem with the login form? see my attached screen shot.

the headers already sent error happens if there is output between php code blocks. see the following thread for details:
http://www.webassist.com/forums/posts.php?id=19285

the error is saying the problem occurs at line 5.

What browser are you using? Looks like its exclusive to IE, where I got this. I have to leave for today, so maybe will pick this up later. Thanks.
