Odd display from CSS Form Builder

I'm building a simple form but I'm getting strange HTML artifacts being displayed when I bring the form up in a browser. I am attaching a copy of the php file.

The artifacts I am seeing are 1) at the top of the form just above the fieldset legend the is a "> being displayed and 2) in each of the menus the values displayed in the drop downs have > in front of the value.

The code is just what was generated but the extension and from what I can see looks to be OK.
