I want to add a drop down box to a page that lists the months of the year with the values being set as 01 for Jan, 02 for Feb etc and then search a date field in a database for months that match the value the user chose from the drop-down.

The form uses the GET method so I've set the filter to be where the field DateCourseCompleted = to a URL parameter also called DateCourseCompleted.
When I click the search button it takes me to a blank search page, rather than posting back to itself, even though the form action is set to PHP_SELF

You have validation using the date validation type. but the values being passed are not valid dates, just the month number.

use numeric validation instead and set the min value to 1 and the max value to 12.
