Universal Email is not functioning

I have Universal Email 401 installed DWCS6.
I created form page years ago and it is still working fine.
Now I created another form and it is not working.
It looks like working and it go to pages set on the UE panel after clicking submit button, but it is nos sending the mail as I don't receive it.
The email is set for pear mail using smtp.
The new form I created is under the same server where the old form is working fine.
I compared the settings of the UE panel and they are exactly the same.
I also found the error log on the server as follows:

I checked the 142 line and it was

require(basename($fileName));

I don't know what that means, but I checked the same files on the working one and it has the same 142 line.

I deleted the confg folder of the DW and deleted the extension and reinstalled, but still have the same issue.

The error means that one of the files needed for the email body is not uploaded to the server.

make sure that all files are being uploaded.

Thanks for your reply.
I uploaded everything, re-checked, but it is not working.
It looks like working as the page returned to thank you page, but I don't get any email.
We have another contact form, under the sub-domain, and it is working fine.
I compared each files and I don't see any difference except one, which is on "PEAR_Mail_using_SMTP_php.php_line 115".
Working one says $mailRef="WA_MailObject", not working one says $mailRef="WAUE".
I replaced the word but no luck.
I am attaching the files just in case. email passwords are replaced with xxx...

I corrected the probelm by uploading an updated copy of the PEAR_Mail_using_SMTP_php.php file.

this is actually a known issue with UE4, there is an updated in your download center.

log into your account, go to the My Account page and click the all downloads link to access the Universal Email 4 downloads, the known issue patch will be available there.

Thank you very much. I appreciate it.
I will check it when I get back.
We are still designing the form, but I guess it will be ok as well as we keep the PEAR_Mail_using_SMTP_php.php file?
Thank you again.
Also I am considering update the extensions.
I am just concerned that if it will be easily transferred to current site that was made by some of legacy extensions?
We are currently using UE/DataAssist/SecurytyAssist/eCart5.
If we updated to new data bridge can we get all the functions above?
(except for ecart, which we have the latest version)
