Adding delivery locations and fees to an existing set up

On our confirm page we have a drop down of several delivery locations that have specific fees associated with each. We would like to add 8 more locations to the drop down (no problem, I know how to do this piece). I cannot recall where I added the individual fees at. I have attached the files. Just a quick reminder would be greatly helpful and appreciated.

it looks like you are using the delivery_fee of the location table:
