File Upload missing

I am running Apache 2.2 with PHP 5.4.16 on a Windows 2003 Server and have a dynamic insert page that is working great. I replaced the URL form element with a file upload and configured the WebAssist file upload tool as found in the tutorial. The page still inserts a record to the database but the file is not uploaded. I've tried changing the path to a full or relative and totally deleting it and I get no errors either way. I've attached a .txt with a URL to see the script in action as well as the actual insert php page.

2 problems:

1) For file upload to function, the forms encoding type needs to be change to multipart/form data.

in design view, select the first form element, then in the tag selector select the form tag to view the form properties in the property inspector and change the encoding type.

2) open the server behaviors panel and double click the Insert Record server behavior to edit it. in the insert record behavior, select the URL field, and change the binding to use the Uploaded Files (url) -> Server File Name binding.

Those are changes that need to be made manually. They will not be done automatically when changing a form element to a file field.

Won't save UR/filename value in database?

Interesting...I just realized that the changes now successfully upload the actual file but the filename value is not longer being saved in the database. It inserts the rest of the data but the URL/filename column is empty? What am I doing wrong? here is a copy of the updated insert page with changes you suggested:
