form quit working

I removed a required field on https://www.surfacespecialistssystems.com/franchise_services/form-shower-pan-measure.php and the form quit working. It also no longer redirects to its confirmation page. This page requires a username and password, which is on the attachment.

  I removed a required field  



if you remove a field, you need to edit the server validation server behavior to remove the validation for that field.

I only removed the validation, not the field itself. I did this to a similar form and it did not quit functioning as this one did.

the submit button has been renamed

The trigger for universal email at line 38:
if ((isset($_POST["SimpleContact_submit"]) || isset($_POST["SimpleContact_submit_x"]))) {

is looking for a submit button or image form element named:
SimpleContact_submit

however the submit button on your form has had it's name changed to SSPan_submit:

<input class="formButton" name="SSPan_submit" type="submit" id="SSPan_submit" value="Submit & Continue" onClick="clearAllServerErrors('SSPan_SurfaceSpecialists_ShowerPanMeasurement')" tabindex="19">


if you change the submit button name, you will need to edit the Universal Email behavior to reset the trigger.
