CSS Form Builder - Form Element List does not accept more than 4 items

When I try to add more than four items (label - value) to a list element in a form using CSS Form Builder I can't. Also the behavior of the Add button changes to that of the Move button. What's up with this?

Is Data Bridge the only Web Assist extension installed? if not what others are installed, including the versions?

Other WebAssist Tools include

In addition to the latest version of Data Bridge (1.1.3) I also have Design Extender (1.1.1), eCart (5.0.4) and Theme-It (1.0.2)

Also I receive the following message when I launch DW:

"Unable to find file WA_Memento.htm in the Configuration/Floaters directory. The floater will not be shown."

the Theme it stand alone extension is discontinued and should not be installed, a version of theme it is included in the Form Builder wizard.

make sure that you do not install eCart last, the install order i like to use is eCart -> Design Extender -> Data Bridge.

the current version of eCart is 5.0.7.

I would recommend uninstalling all of the WA Extensions you have installed, then reinstalling the current version of eCart followed by design extender an data bridge.

also, reset the workspace layout in DW to correct the floaters error:
go to Window -> Work Space layout -> Reset '<your layout name>'

Same error........

No sooner after upgrading I get my first bug within 10 mins of use if fact no make that 2!

1) Adding Form Elements List label and values, After adding the 3rd item all the fields go blank removing the previous entered content, however the fields are still active (see image1) looking at the source code confirms this :-

I have tried following the previous recommendations but got the same result, I uninstalled all the webassist extensions, deleted the cache, rebooted and then installed my extensions: Free Tools 1.0.2 > eCart 5.0.7 > Data Bridge 1.1.3

2. When adding entries (Item Label & Value) you must hit the enter key after entering the value on each line and not click the add icon as this removes the entry, this may sound minor but it is very annoying.

I was reluctant to upgrade to this extension as I had already had the extension but due to upgrading to CC CS6 I had no choice to continue using it and after running into quite a few issues with the standalone form builder before DB which lost me good customer due to continues errors from test builds addressing validation issues. I cannot believe a simple requirement of the form builder is not working!

I am not able to reproduce the problem that you are reporting.

Do you have other WebAssist Extensions Installed? if so what version numbers?

Have you tried Deleting the Dreamweaver Cache file from the users configuration folder:
