Help with Show Region Behavior

I being trying to protect portion of a page with SecurityAssist ShowRegion behavior. I have created a rule named "Survey" where I have set the following:
Value: 1
Criteria=
Compare to: <?php echo ((isset($_POST["code"]))?$_POST["code"]:""); ?>

On my index.php page I have one input box that redirects to survey.php page. The form shows only when the code of 1 is entered.

Before I added Universal email to the survey.php page, show region server behavior was working fine. After the code of the Universal email was added, the following happens:

When I entered the value of 1 or any other value on the index.php page, I get redirected to my thankYou.php page. It bypasses the survey.php page.

I am using Windows 7 pro DW cs 6 and the latest WA extensions.

On the index page, ,the name of the form submit button is "button"

on the survey page, the submit button also named button.

in the universal email behavior, you have the trigger set to Button: button pressed.

this is causing the form on the index page to trigger the UE behavior.

to prevent this, change the submit button name on the index page.

Can you please me help on how to decipher these error messages

I added the following code at the very top of survey.php

delete the webassist/email/mail_php.php and webassist/email/mailformatting_php.php pages from your site, then open the server behaviors panel and double click the Universal Email server behavior, and click the OK button to have those 2 files regenerated. make sure to upload the new versions to the server
