Errors when creating Security Assist pages

I followed the tutorial on creating Security Assist pages, but get the following errors when they're being created:

what extension including the version numbers are installed.

have you tried Deleting the Dreamweaver Cache file from the users configuration folder:
common_installation_issues.php
