Form Styles

I sometimes create a form in DataBridge and when the form is produced it appears as a single column as shown in this example.
I would like to get the sub label on the same line as the text box.

I am fairly new to css and would be glad of some help as how to do this.
I am attaching a picture of how the boxes look and also a zip of that page

I would need to be able to see the form in the browser in order to troubleshoot the css.

Now theres a thing. When I open that page on a remote server the page appears correct. Strange or not so?

most likely, the CSS for the page is not uploaded to the remote server, so whatever css is causing the width of the form to be narrow, is not in place

Do you have any idea where the css file may be?
I uploaded all in the webassist file but nothing changed.
