WA Validated Entries bindings missing

I have inserted the Server Validations behavior to validate the Captcha code. This is working fine but of course all the data in the form is lost when the form error is returned. I know you have answered this question many times and in the help file it says 'In the Bindings panel, select the WA Validated Entries binding that correlates to a form element on the page'. But, I don't have that in the bindings tab - please see attached image. I am using Dreamweaver 5.5.

have you tried Deleting the Dreamweaver Cache file from the users configuration folder:
common_installation_issues.php


if you continue to have a problem, please send a copy of the page.

Yes, I've tried deleting the .dat cache file, but still no WA Validated Entries in the bindings. I'm attaching a copy of the page.
