Update Record JavaScript Error

I just updated my Data Bridge and went to create a page where I could update a record. When I click on update I get error:
While executing onLoad in WAAB_UpdateSBV3.htm,
the following JavaScript error(s) occurred:

In File "WAAB_Update_SBV3";
initializeUI is not defined
"

I also deleted cache several times.

try Restarting the computer and delete the cache once more

I removed cache and restarted the computer. It is still prompting same error on screen when I click update single log on.

What extensions including version numbers are installed?

was data bridge the last extension you installed?

Data Bridge must be installed after all of the others. try uninstalling Data bridge, then delete the cache file and re install it.
