Date & Time Issues With Security Assist

I am attempting to create a registration page with Security Assist, but I must be making some basic mistakes because I cannot get the Date and Time fields to work properly.

In Security Assist:

The DATE column is displayed as TEXT
The validation type is set to DATE
The format is set to mm/dd/yyyy
The DATE column in SQL is using data type DATE

I also tried displaying the datepicker in the Date Column, but that did not work for me either.

The TIME column is displayed as TEXT
The validation type is set to TIME
The format is set to hh:mm tt
The TIME column is SQL is using data type TIME

Are these settings proper or do I need to do some additional coding?
This seems logical to me, but I error messages when I do it this way.

what are the errors? I need to know the errors you are getting in order to help, without knowing the error message, it is impossible to tell what may be wrong.
