Insurance Forms Library call to:

I have multiple "privately owned" insurance web sites. I have a question regarding creating a "master" forms "call-to" library of specific forms. Currently I am working on building a web form for a contractor bond. Approximately five of my insurance sites will allow the users to click on find a bond. I am trying to avoid having to make a database for all five sites.

I envision having to make up to several dozens of variations of forms / applications (just for the bond types)

Then, similar to the bond forms I will need it for various other classes of insurance. Here are some examples:

  • General Liability for "hundreds of classes"
  • Workers Compensaiton
  • Professional Errors and Omissions
  • Cyber Liability
  • Farm and Ranches



Along the same train of thought the forms would need to have optional sections to opt. to complete or required to complete depending on the response.

Finally, the form would need to have expanding table rows and columns.

What suggestion or suggestions would you recommend? I own all the sites and could set up a database on just one site and allow access to the forms library from the other sites.. (some how? How?)

As a final suggestion, my eventual goal is to set up a domain where I could sell subscripton to a search for form function for other insurance peeps.

Thank you for your assistance. I am seeking a solution for the long run and have minimal finances so I am trying to use sweat equity to do this myself.

I'm not sure specifically what your question is.

Dreamweaver can be used to create forms, or you could use CSS form Builder from Data Bridge.

CSS form builder can be used to add validation to certain parts of the form, and you could use the insert record behavior of Data Assist in data Bridge to store the information to a database.


  the form would need to have expanding table rows and columns.  



you would need to use Spry Accordion to accomplish this part.

Another angel starting small

Similar to "your wa CSS Form Build: Add form which has two tabs. One for Form Details and one for Form Function. I need to have a "form" for homeowners. Eight tabs. Each tab would have a "table?" with up to 20 rows. How do I go about using form builder to do this?

yu would not be able to use form builder to create a form like this, you would need to create the form manaully.


add a form to the page:
insert -> Forms -> Form

then add a spry tabbed panel inside the form,

then you can add form elements inside each of the panels.
