Authenticate password request email

I've a password reminder page and field setup.
If a user submits a valid email which is present in my table, an email is sent to the requester containing the password reminder.

How do i validate the email address on submission?

If the email exists in my table then the process continues.
If it does not, then an error page is displayed instead.

On step 2 of the Email Password Server Behavior wizard, there are two redirect options:

If Look up matches, Go To:

If Look up differes, Go To:

if the emial that was entered is not in the database, the user will be redirected to the page specified in the If Look up differes, Go To section.
