Problem displaying contact form

Nearly finished building a site for a client but having a problem with creating a contact form using CSS Form builder 1.0.1 from separte SuperSuite extensions. Done this successfully before on another site some time ago.

The coding for the form appears to be present in the coding view but in the display view and on the live server, the form simply shows a small block headed with "Contact me".

Have tried to sort this for a while by working on a temporary copy and taking away any containing divs and other parts from the page but still wont show the full form. The form doesnt seem to be enclosed by the <div id="SimpleContact_Cool_Default_ProgressWrapper">.

Just can't see where the problem lies, hope you can help.

in the webassist/forms/fd_cool_default.css file, change:

form.Cool_Default ul.formList {
display: block;
list-style-type: none;
margin: 0;
padding: 0;
}

to:

form.Cool_Default ul.formList {
display: block;
list-style-type: none;
margin: 0;
padding: 0;
display: block;
}

Good progress, the form now displays but only shows the content blocks without any labels, captcha etc.

add:

font-size: 11pt;

I use ie9, firefox 16, crome v23 and safari 517 to view the site as its built.

I include the four screenshots. I could get firefox and safari to display it somtimes by repeatedly refreshing the page but no amount of refreshing makes it visible in the other browsers.

Just noticed that safari is not showing the labels
