data assist: insert page error message

Hello

I have the insert page working before and I don't know what I did and I started having this error.

I have uninstalled the extension and re install it but I am having the same error.

While executing onLoad in WA_DFP_fileUpload.htm, the following javascript error occurred

in file "WA_DFP_fileUpload" initializeUI is not defined.

I don't know what I am doing wrong but I keep on having one error after another. I Prefer pay for the ticket and have someone show me what I am doing wrong. I will save time and money than looking in the dark for so many days.

Delete the Dreamweaver Cache file from the users configuration folder:

Thanks for the support - it worked.
1. The insert page

I have tried again and having these problems and need help. When inserting image I have the error message I do not understand. (attached message )

2. The detail page

My detail page came up with a black bock with the delete button inside even though I did not select the delete option. (attached page)

3. The result page

I know this is the css problem but I can not find it. Some of the blogs test are in the specified size and fit on the page and some overlap the page. ( You are not here to fix my css problem and I know you are have a lot to fix in one day. ) (attached page)

I have created a support ticket so we can look into this issue further.

Jason had followed up the ticket created and tell me where was the problem with the insert page but I am still having this problem I have mentioned with the detail page and I have forgotten to ask him when he was working on the insert page. (That was my #2 question)

2. The detail page

My detail page came up with a black box with the delete button inside even though I did not select the delete option. (attached page)

Even if I select the delete option the outcome page is wrong with the big black box at the bottom of the page.
