DataBridge 1.9 Server Validation FAILS

INSERT FORM build using 1.0.9. fails on the server validation. I can't find any tutorial or instruction on how to debug this error. The insert form just reloads a blank page. If I remove the validation it works so the validation is not being written completely.

The attached files are unedited and the insert form doesn't work, neither does the update.

a blank page means that a php error is occurring but error reporting is turned off.

to turn error reporting on, add the following code at line 1:

php:
<?php

error_reporting
(E_ALL);
ini_set('display_errors','on');
?>

once we know what the error is, we will be able to troubleshoot the cause.

on the update page, the connection line is in the wrong place.

move the following code from line 47:
<?php require_once('../../Connections/conn_asbs.php'); ?>

to line 1
