Inventory isn't being kept up to date

Today, I received this message from client using PS 4:

"Yesterday I added a short online with an inventory of 2 for small black. This morning I noticed a women purchased 2 small black shorts last night, so out of curiosity I checked the inventory and it said there were 2 small black shorts available. I expected it to say 0 for small black. I just called (office personnel) to ask her about it and she said that she has had to go in and manually adjust the inventory. She noticed about a month or two ago.

I ran a test. I purchased a shoe that we only had 1 available in. I then checked the inventory and it said there was still 1 available in that size."

Any ideas where to look to correct this issue? Same store as other support tickets.
