Hash Mismatch & logo design issue

A client is getting the following error:
Hash mismatch
Reference ID: 200509824

She had a credit care and $1500 to submit but can't. Store is 4.5 and located at powerstore/

I've not been able to recreate this error. I have been able see an issue with the logo on the site in the design not sticking.

Please advise soonest
