Code for attaching file

I am trying to attach two files to an email after they have been uploaded from a form. I have read other posts where you have explained what to paste into the value using the Universal Email behavior. However, I'm having an issue with my server behavior working correctly. When I click the attachment icon in the toolbar all I get is a blank screen (see attached). Can you tell me the actual php code I can add/change to accomplish attaching files after they have been uploaded?

it would be better to fix the white screen so you can using Universal Email to create the correct code.

have you tried Deleting the Dreamweaver Cache file from the users configuration folder:
common_installation_issues.php

if that is not working, what version of UE are you using?

What is your OS?

What is your Dreamweaver version?

What other extensions, including version numbers, are installed?

I tried that and am still having the same problem. I have the latest Super Suite and have installed the three extensions in the correct order multiple times. I had previously installed them out of order and so I had to reinstall. I'm running CS5 on Windows 7. The only other extension installed is from Adobe for the HTML5 pack.
