This happens if you are selecting to use a remote checkout method like Paypal Payments standard.
The email receipt options will only be available when using a local gateway.
using a remote gateway, the gateway will handle sending the receipt

this is probably a dumb question, how will i know when i get an order, will the gatway notify me as well?
In your account settings for the gateway, you can configure email alerts to have them send an email when an order is placed.

I want to send an email to the customer once they have placed their order - on which page should I add the UE and from where do I get the values needed?

What gateway system are you using?
in order to send a receipt using Universal Email, you need to be using a Local Checkout method.
When you run the checkout wizard, and you select to use a local checkout method, you will have the option to send a receipt. if you use a remote checkout method, the payment gateway will handle sending the receipt when the transaction is processed.
the reason for this is that remote checkout systems do not process the transaction in real time

World Pay does not offer a local checkout method, it only offers a remote checkout method.
you will need to let worldpay handle sending the receipts.
