Multiple Records being returned in search

I used data assist to search a database and it returns multiple copies of the same record.

CPO_Name.php and tpye in "Da Puzzo" for an example.

the files you attached are not the original php files?

I need to see the PHP files that have the recordset for the results page in order to inspect the code.

The code on your page looks correct, it must be that there are multiple records in the Certified_Planning_Officials table where the last name column has the value Da Puzzo,

I'm sure if you where to access the MySQL Database using PHP MyAdmin or another MySQL GUI utility and run the following query:

SELECT * FROM Certified_Planning_Officials WHERE Last_Name LIKE '%Da Puzzo%'

you would get the same results.
