WA_Appbuilder.htm

I am using CS4 and when using the DataAssist Wizard get the following message:

While executing onLoard in WA_AppBuilder.htm, the following JavaScript error(s) occurred:

In file "WA_AppBuilder":
initializeUI is not defined

This product worked before, and I have attempted to reinstall, but with no luck. Any ideas?
