At this time CS6 is not a supported platform, for stand alone extension.

Wer have been testing Design Extender and Data Bridge for CS6 compatibility, and can report that those extensions will work in CS6.

I would recommend upgrading to Design Extender or Data Bridge in order to use the extensions in DW CS6.

when you purchased CSS Form builder, the system requirements stated that is compatible with Dreamweaver CS5, Dreamweaver CS4, Dreamweaver CS3. There was no mention of CS6 compatibility at the time you purchased.

The stand alone versions of the extensions will not be updated to work in DW CS6.
