Second form on same website

I created a form with the Form Builder and it is currently working fine:
affiliates.php

I went to create an additional form on the same web site and I cannot get it to work.
service_request.php

When I created the new form, it overwrote the formatting on the first form and the Form Builder doesn't seem to be adding all of the code needed to validate the forms. It seems to be missing code each time I try to build it. I've gone through the Form Builder wizard multiple times but I keep getting the same errors.

Would it help if I changed the name of the first webassist/ folder so it doesn't overwrite the first one and it adds all of the code that it should to a second webassist/ folder? I'm stuck and can't move forward... Please help!

I'm beginning to wonder if there's a problem with my software. I've created about 4 or 5 forms successfully but now I am having trouble. Now the problem seems to be bigger than just the one web site.

I tried creating a new form on a completely different web site today after the issues that I was have listed above.

order.php

This new form is also having errors that seem to be from the Form Builder not properly entering code as it should. Once I created the form, I got another syntax error for line 237, 260, 288, 311, 346, 422, 445, 468, 491, 524, 672, 706, 729, 752, 814, 839 and 916. All of these lines have the same code:

235 <?php
236 if (ValidatedField('order','order')) {
237 if () {
238 if (!(false)) {
239 ?>

This is the problem line of code for each syntax error. Why is my Form Builder not entering the proper code and causing these errors? I had no problem for the first 4 or 5 forms that I created. Now all have these same errors or similar issues. HELP.. AGAIN!

What version of form builder do you have installed, make sure you have installed the latest version that is available from the download center, if you have a previous version installed, make sure to uninstall it from the extension manager before installing the newer one.


when creating the second form, if you use the same design as the first form, but make changes to the CSS, make sure to save the changes as a new design so it will use unique css file naming and not over write the css files for the existing design.

if you change the design, but do not save the design as a new one, it will over write the old one.

I bought the Super Suite Jan 25th, 2012. I updated with the 201 update before I even began using it for the first time. Isn't that the most recent update?

Thanks for your response about the CSS, I understand that now. But I am still having major problems with the form even functioning.

What do I do?

What other extensions do you have installed (including the version numbers)?

This may be a conflict issue, see the following thread for some install tips to eliminate extensions conflicts:

I have all of the Super Suite extensions installed. When I purchased the software, I installed it as was described. I did the version 2.0.1 of the Form Builder first then the HTML Editor 3.

How can this be a conflict or installation error when it was working fine for 4 months? This is a new issue that just started without making any changes or adding any new extensions. I have created several working forms but now none of the new ones will work. What can I do to find a solution?
