How to add a field to the INSERT RECORD FORM

Hi There,
I would like to add 2 new text fields to an existing Insert Record Form.
Instead of recreating the whole form using the wizard, I have inserted two text fields into the form, but I can't work out how to connect them to the database.
(At the moment when testing, any data entered into these fields doesn't show on the database).

I have looked through all the Server Behaviours, but nothing seems to be obvious or work.

Please can someone point me in the right direction.

In the server behaviors panel, double click the insert record behavior. on the second step of the insert behavior wizard, select the column for one of the form elements, then click the lightning bolt next to value and select the form element ,repeat for the second element.
