SA Wizard Forgotten Password Page Issue

Im using SA 2.0.4 on DW CSS5.5

I am using the wizard to create a PSWD Reminder page.

During configuration of the password reminder page, it appears to fail to show the 'all important' email field in the preview. Instead only showing the security and CAPTCHA questions.

Screen-shots of steps attached.

Appreciate advice on how to ensure the Email field is given and only the CAPTCHA element is included - I don't need the security question.

Again the question of SA's feature-customisation is raised.
I'd like to see the Wizard support more options to include/exclude features, allowing users more control over page appearance and functionality.

This appears to have been resolved after removing CSS Form Builder 1.0.2 and Cookies Toolkit 3 from my active extensions. Then removing SA 2.0.4 completely, wiping the CS5.5 winfilecache and re-installing SA again.

Then building the form from scratch.:mad:

Possible conflict??
