SA Wizard "Error in populate.getFromFormBuilder" Message.

SA 2.0.4. in DW CS5.5

After configuring my security form pages using the wizard, the process runs to create and immediately fails-over with the message;

Error in populate.getFromFormBuilder:
Type Error: This.src.lines[opts.lineIndex -1] has no properties

Images attached in order of events.

Any ideas on how to resolve as cannot continue?

This appears to have been resolved after removing CSS Form Builder 1.0.2 and Cookies Toolkit 3 from my active extensions. Then removing SA 2.0.4 completely, wiping the CS5.5 winfilecache and re-installing SA again.

Possible conflict??
