problem with redirect after insert record

have Data Assist 2.08 and just upgraded to DW CS5.5
using DA to insert single record and the problem i'm having is with the redirect (the insert is working properly)
The problem is not redirecting after the insert.
In IE it shows not found and in Firefox, i get this error message
The requested URL /var/www/vhosts/casaolepensacolamexicanrestaurant.com/httpdocs/thankyounewsletter.php was not found on this server.
Apache/2.2.9 (Debian) PHP/5.2.6-1+lenny13 with Suhosin-Patch mod_ssl/2.2.9 OpenSSL/0.9.8g mod_perl/2.0.4 Perl/v5.10.0 Server at www.casaolepensacolamexicanrestaurant.com Port 80

thankyounewsletter.php is the page for the redirect and it is on the server?

here is an updated copy of the WA_DataAssist/library.php file that should correct the issue
