Booking Form

I've created a booking form (booking_form.php) using CSS Form Builder 1 and Universal Email 4. When I click submit I receive the following message:

Notice: Use of undefined constant undefined - assumed 'undefined' in /clientdata/zeus-dynamic-1/t/s/tsvllc.com.au/www/booking_form.php on line 16

Warning: Cannot modify header information - headers already sent by (output started at /clientdata/zeus-dynamic-1/t/s/tsvllc.com.au/www/booking_form.php:16) in /clientdata/zeus-dynamic-1/t/s/tsvllc.com.au/www/WA_ValidationToolkit/WAVT_Scripts_PHP.php on line 71

When the submit button is clicked it is suppose to go to a 'thank you' page. Can you please give me some advice as to what is wrong here and how I may correct the problem.

Thanks Jason. This seems to be working now.

The client has now asked me to add the file upload feature to the form. I select the form and click 'edit contents' and add it to the form. I think I have done this correctly. When I edit the element to make it not a 'required field', it does not allow me to do this when I uncheck the 'required field' option.

Also, Webassist instructions say, if your form contains a file field, the 'Form Upload Settings' option will be available on the 'Form Function' tab. I am unable to find the 'Form Function' tab. Is this all I need to do to make my file upload feature and the rest of the completed form fully function?
