css from builder pages crash dreamweaver

For some unknown reason pages with css form builder 2.00 forms installed are crashing dreamweaver. no error message is given from dreamweaver but i get the vista error "a problem has caused dreamweaver to stop working"

I compared the forms to an older backup version and they are identical. The forms work on the live webpage. I tried upgrading to cssform builder 2.0.1 and the issue persists.

other pages without the forms open ok.

I can open the css from builder wizzard on a new page without it crashing.

Your help in resolving this would be appreciated.

Had same issue

Had same issue but remedied by making sure I saved and closed newly created php file. Open it back up after saving and form builder did not crash DW 5.5
