Insert Record is replacing instead of adding

I have a simple page that is supposed to upload a photo and then add a record to the database with the file name and unique ID. The upload works great, but the insert record behavior sometimes will add a record and sometimes replace the last record, (nothing happens to the file though).

An insert command cannot overwrite an existing record. It can only create a new record.

Are you looking at the information directly in the database to determine that it is being replaced? or does it appear like it is being replaced in another page in your site?

I would suspect the later to be the case, and i would suspect that the page where you are seeing the problem doe not have a repeat region to show all of the records.

I DO have a repeat region, I AM checking the DB directly, it IS replacing the previously added record.

The thing about it is that it always works just fine the first time you try it (with a new browser session). After that it will randomly work correctly and incorrectly. I have a hunch that it has something to do with some session variable, but no way to say which one, or any idea where to start trying to find out! :-(
