Update upload works but not Insert

I've added upload file functionality to DataAssist created pages. The upload works on the update page, but not the insert page. Both are putting uploads in the same folder with the proper permissions set. The debug_info() shows an error code of 0, no file uploaded.

I've attached both files as a zip in the hopes you can set me straight.

on the insert page, edit the form tag:
<form action="press_Insert.php" method="post" name="WADAInsertForm" id="WADAInsertForm">


to:
<form action="press_Insert.php" method="post" enctype="multipart/form-data" name="WADAInsertForm" id="WADAInsertForm">
