UE4 / File Not Attaching

UE4 / File Not Attaching

I am trying to upload a file to my form and submit the form to an email. The form submits just fine, however, no attachment. I have done these before without a problem... not sure what I am doing wrong. I have followed the webassist instruction, set my form to multi-part, etc etc. The form is processing thru a Go Daddy server... not sure if this complicates things. Please take a look at my attached file.... and let me know if you have any suggestions.

In universal Email, go to the attachment settings, make sure to select the Entered Value option and use the following code:

UE 4 not sending attachments

I am using 1and1 for my web hosting. I copied and pasted the code but the email form is not being able to send the attachments.

Attachments are being loaded but attach to emails

I noticed that my form was missing the following enctype="multipart/form-data". I added the code and the attachments are being uploaded to the folder I assigned but not attach to my emails. I just want the attachments be attached with my emails. Can you please help.
