Jason,
First off I wanted to thank you for all of your help on the phone last week. after we finished, I made some changes to the database naming convention as you suggested. After that, I re-ran the Data Assist Wizard. I am now getting the following error message after I insert data and click "Insert". The requested URL "/Applications/MAMP/htdocs/NOS/tblAccountInfo_Detail.php was not found on this server".

The form is submitting data to the database, but it appears something is still not provisioned correctly. Any ideas? I can send you screenshots if you don't remember how it's configured. I'm using a MAC with MAMP and the default settings. Thanks

there is a fix for this issue in the known issues section of the data assist download center page.

Do you have the correct path for MAC OS Lion and Dreamweaver 5.5? It appears to be different.

correct path to what? the configuration folder?

it should still be the same, though it looks like the library folder is now a hidden folder:
cpsid_90508.html
cpsid_90947.html

ok, so I was able to locate the library folder and replace the library.php file. I then closed dreamweaver and relaunched, but got the same error message after click "Insert" on the form. Do I need to re-run data assist wizard? My current library file is dated 12/22/10.

try commenting line 11, change:

php:

if (function_exists("rel2abs")) $WA_redirectURL = $WA_redirectURL?rel2abs($WA_redirectURL,dirname(__FILE__)):"";

to:

php:

//if (function_exists("rel2abs")) $WA_redirectURL = $WA_redirectURL?rel2abs($WA_redirectURL,dirname(__FILE__)):"";

Bingo... that worked... thank you... can you explain why that happened? Should I have done something different, or will I have to remark this line out all of the time?
