issues with 2nd form on website

Hi all
i am having an issue with making my 2nd form on my site work.

i have 2 different forms on 2 pages,
1 form on one page works correctly and sends to the address specified correctly.
the 2nd form appears on the webpage, enables me to complete the filling of the form and when i press submit form i am given a new screen popping up in my browser with the following error

the form does not send.
Any help will be greatly appreciated

The error is saying that the webassist/email/tempaltes/Blank_9.php file is not uploaded to the server.

make sure to upload the entire webassist/email folder to the server.

i did that and it didnt work, but on the 2nd upload all works well,
