Radio button value being ignored - not inserting into DB

*********UPDATE: Fixed the issue - sorry - I didn't see there were 2 insert actions on the page, the 2nd one was causing the error***************

Hi,

I have a form with a list of products on in a radio button group. The idea is the user chooses a product by selecting a radio button. However, when I try to insert a record from the form the value from the checked radio button is being ignored because I get a message stating that ProductID cannot be a null value in the database table.

My radio buttons all have the same name and are correctly pulling their dynamic values from a recordset (checked by viewing the source of the page).

I've attached a copy of my insert-record page. Any ideas as to why this isn't working?
