Datepicker not functioning

When I bring up the date picker to change the style and click the OK button the date picker does not execute, nothing happens. I have both cleared my site cache and deleted the DAT file. I have also deleted and reinstalled Form Toolkit.

The date picker was inserted when I used CSS Form to created the form.

I am user CS 5.5

I am also having a problem with the date picker and IE 7, 8. The date picker functions when the page initially loads. But when you click in the date picker field again IE 7, 8 shows an error and the date picker quits functioning. Moreover, the Best Times field next to it also quits functioning.

Internet Explorer is generating this coding error...
tBlur = tBlur.replace(/\bthis\b/g, "docElm");

Also when the error is thrown Internet Explorer amends the URL and adds a # sign to the end of the path, his seems to kill the date picker.
For example when the date field is clicked into the URL was index.php this and is changed to this index.php#

You can view the problem at the following location...
index.php
