Post data from form

This page submits data to the database and I have universal email on it. I am using a lot of hidden fields. uploads.php
When submit is clicked, it sends the form and images to database.

How do I get the data to post to my email? I tried to do it with binding with the form I only get the 4 fields from this.

Also, is there a way to send it as a pdf. I am not sure how to do that.

None of your hidden form elements are inside your <form> tag. You'll want to move them above the </form> so they can be accessed in the email body.

You can use php to create a pdf file if you have the necessary components installed, but we don't offer any extensions or examples of how to do that. You would have to hand code that, but once it is created you could attach it to the email pretty easily with Universal Email.
