Security Assist login error

Hi all,

Used SecurityAssist 2.0 to create registration, login, update etc... pages. Registration works fine, but the login page comes up with the following error when username and password has been entered...

PHP Notice: Undefined index: Log_In_group_Remember_my_information in C:\inetpub\wwwroot\project\login.php on line 19 PHP Notice: Undefined index: Log_In_group_Log_me_in_automatically in C:\inetpub\wwwroot\project\login.php on line 39 PHP Notice: Undefined index: Log_In_group_Log_me_in_automatically in C:\inetpub\wwwroot\project\login.php on line 44

This was a freshly created php site using DW CS4. I configured the wizard to use an existing database though.

Any ideas what it could mean?

the error means that that there is a reference to a variable named Log_In_group_Remember_my_information that does not exist.

send a copy of the page in a zip archive so I can examine the code to tell anything more than that.

Attached is the login.php and screenshots of errors that occur when running the wizard. I have stripped down all Dreamweavers extensions (A couple of dozen!) and still receive the issue. Another problem with running the wizard from my pre-constructed database appears to be when I select postcode validation for the UK (shown in the screenshots). The postcode field doesn't even appear in the form.

Would it also be possible to provide a step by step (aka idiots guide) for a double opt in setup, as I have spent a lot of time with this issue rather than figuring out the opt-in, and I know a lot of people would find it extremely useful (We need it for insurance purposes)?
