DataAssist Multiple Search Issue

I have 3 search forms on the attached code. One is a general search, the other 2 are filtered dropdown searches. The dropdowns work perfect but the general one doesn't work at all. If I remove all info regarding the dropdown searches the general one works perfect. I need all 3 to work and I need them to reset when the page is unset. Any ideas why this not working? This is my second time to submit this thread but somehow I didn't post the first time?

please dont paste the code into a text file.

compress the original php files into a zip archive and attach the zip[ file so i can see the complete code in context.
