Cannot redeclare validatedfield

Hi. Struggling to add Captcha to an existing WADA registration form (with check username). I've tried using FTK and rebuild in WAFB.

Among other errors, this one keeps popping up:

Fatal error: Cannot redeclare validatedfield bla bla ....

I think I need to delete some server code. And maybe these is an issue with old/new WA validations - I can see the form is using more than one WA folder. Any ideas?

1) delete line 4:
<?php require_once("WA_ValidationToolkit/WAVT_ValidatedForm_PHP.php"); ?>

2) Do not use 2 instances of the Server validation to add captcha. Instead, edit the existing server validation to add the like entry captcha validation to it.

3) You will need to remove all of the Validation show if behaviors and re apply them.
