Email fails every time... HELP!

I'm trying to configure my form to email me with an attachment upon submission. I can't seem to figure out why it fails everytime. I attached my php pages and notes which include yahoo hosting email settings and my file activity log which shows an error on upload.

Thanks!
Amy

the smtp server you re using requires authentication.

The PHP Mail object does not support connectiong to an SMTP server that requires authentication.

For this you will need to use the Pear Mail For SMTP mail object, you set this on the SMTP settings tab of the Universal Email wizard.

you will need to contact the host to determine the correct path to pear to use.

Apparently the problem was that I needed to set up PHP/Perl Mail on my yahoo site and use the "From" address (that I set as the default in this setup process) in the mail function. I was trying to make the "From" dynamic which I think was my biggest issue. Apparently that's not possible. Thanks!!
