Error Creating form with FormBuilder 1.x on PHP

I have FormBuilder 1.x (Latest published release). The extension works well when inserting a form under html files. I tried to insert a form on a PHP page and got many errors (some of which I am attaching snapshots of), at the end I got a final error with a message saying it could not find the Javascript code for a widget (also attached a snapshot of it).

Please help.

here is an updated copy of the Shared\WebAssist\FormBuilder\shared.js file that should correct the issue, please copy if to the Shared\WebAssist\FormBuilder\ directory in the users Dreamweaver configuration folder, see the common installation issues pdf for help locating the configuration folder:
