Lots of errors

Just got around to installing CSS Form Builder 1.02.

Removed Validation Tool Kit
Installed extension.

Created form with wizard, made a few formatting changes.
clicked Finish.

Errors captured to image can be found here.

Is this extension current? Has it been amalgamated with something else?

I've experienced problems with form wizard, form configurations css editing, storing settings, and the forms created do not work properly when tested

really need urgent solution!!!
(i've already tried to unistall/reinstall - uninstall takes a long long time to perform if compared to install time)

Deleted cache
opened page in dreamweaver, got the same Spry/Javascript warning.
Created new page, inserted form again.
Same errors exactly.
